In most cases, women facing breast cancer surgery will have several breast reconstruction options. Understanding these options before surgery is very important in deciding on a surgical approach in concert with your cancer care team.
The direct to implant (DTI) breast reconstruction procedure provides women with a way to wake up from their surgery without ever missing their breasts. This immediate form of breast reconstruction places a breast implant within the breast at the same time the mastectomy is performed in order to create a beautiful, natural-looking appearance of the affected breast or breasts.
The tissue expander approach for breast reconstruction is performed as a staged procedure to stretch the breast skin and tissue for placement of a permanent breast implant. Tissue expander reconstruction is currently one of the more common approaches and allows most women the option of implant-based reconstruction even if they are not left with enough breast skin at the time of their mastectomy.
A number of techniques can be used to achieve nipple reconstruction. The “skate flap” technique requires very little alteration of the overall shape of the breast. A special incision pattern is used to cut an area of skin and soft tissue, which is then folded to create the nipple. This skin and tissue is then replaced with a skin graft from another part of the body, such as the upper inside thigh, to create an areola.
When breast reconstruction is performed at the same time as your mastectomy or lumpectomy, it is known as “immediate” reconstruction. Breast reconstruction that is performed weeks, months, or even years later is known as “delayed” reconstruction. In most cases, you will have a choice as to whether you want an immediate or delayed reconstruction procedure.
